[wp-pro] Extra fields to comments form

Hello guys,

 

Has anyone added extra fields to comments form ?

 

I'm trying this on 2.9.2 version.

 

I've tried this:

Followed istructions in comments as well, but it doesn't seem to work for
me.

 

Any ideas ?
