[wp-pro] need a contact form plungin

I'm currently using WP-ContactForm 1.1 by Ryan Duff
  It doesn’t suit my needs anymore
   
  I need a subject line (Possibly a drop down list of subjects)
   
  It would be nice to have a drop down list of categories that would send the email to different email accounts.
   
  I want more info from the sender (phone & address options)
   
   
  Let me know what you can do, how long it should take, and costs.
