WordPress is evolving so quickly making parts of the Codex out of date with every release. It's hard to tell what information is 'correct' or current, ESPECIALLY when you are new to WP and relying on the Codex to show you the way. 

I'd love to see clear indications of both the WP version the info was written for, as well as when the entry was written/updated.

That might be able to ease upkeep as well if reminders to the author(s) could be set based on version or date. For instance, a reminder could be sent out to check/update content on WP version change or after six months or so.
