Looks like he's (quite intuitively) clicking on the image icon in the  
main tinymce toolbar, which doesn't provide a Browse function, but  
expects a URL. To browse to an image (surely this is the majority use  
case) you have to click the first Add Media square.  I can see why  
he's confused.

I'd suggest removing the image icon from the toolbar altogether -- the  
Add Media dialog already has a "From URL" field. The Add Image button  
on the main toolbar only serves to confuse.
