I happen to stumble on a little plugin -> WordPress Internal Rewrite Viewer Plugin which gave me enough clues to get this working..

One issue I did find was when WP Permalinks are set back to default, WP leaves the rewrite rules behind in the .htaccess and more important no rewrite filters are being called which is a very quick way to = broken URL's.
