[wp-docs] Template Hierarchy Diagram

At this point, all I want to do is update the diagram. I don't see any
glaring issues with the page text. :)

As for "too busy": I could certainly make a version without the conditional
tags and body_class CSS classes, which would cut down on the busy-ness.
