[wp-docs] Template Hierarchy Diagram

Ouch, somehow I missed this in my email box. Part of the joy of traveling,
it takes too long to catch up.

The template hierarchy graphic has been hot linked to and used on a LOT of
external sites. Is the original still there on the Codex or was it deleted?
I'm still trying to follow the conversation. If it was deleted, we need to
put it back but we can remove it from the article. It would be nice to put
a note on the graphic that it was updated with a link to the page.

The new graphic is excellent. Well done! Thanks for the fantastic update.

I edited the example section to break the huge paragraphs into ordered
lists to help improve readability.

Do we really need the changelog? Can that go on the page's discussion? Is
it important to leave there? I'm seeing changelogs popup around the Codex
and I don't remember that discussion. Did I miss that one, too?
